Renewal Process - Perfusionist

Senate Bill 202 transferred four occupational regulatory programs from the Department of State Health Services (DSHS) to the Texas Medical Board (TMB). Medical Physicists, Medical Radiologic Technologists, Perfusionists, and Respiratory Care Practitioners will now be regulated by the TMB and each occupation’s respective board/advisory committee.

 

Due to building-wide electrical maintenance, there was a service outage starting at 5:00PM on Friday, Feb. 19 through 8:00AM on Monday, Feb 22.  During this period many TMB online services, including License Application, Registration/Renewal and “Look Up a License” were unavailable.

Previously, if your DSHS permit expired in December, January or February, you were given until the end of February, to complete your renewal/registration.

Due to the service outage mentioned above, DSHS transfer permits that were set to expire in December, January or February have been extended again, and you now have through March 31, 2016 to complete renewal/registration (including fingerprint results) with the TMB before your license becomes delinquent.  Your TMB permit expiration date has been updated to reflect this extension.

No further expiration date extensions will be given.

The TMB began processing the registration/renewal of existing licenses on January 4, 2016.

 

 

PLEASE NOTE:

  • If you have an existing license or permit that was issued through DSHS and need to request an upgrade, you will need to submit a new application, using the online system located in the Licensing section of our website.
  • Current rules for these occupational programs will continue to be in effect until amended by TMB, as directed by S.B. 202.
  • Please keep checking our website for updates and additional information regarding the transition of this registration process.
  • As part of the transition of this renewal process, new renewal forms have been created, including new questions.  In addition, renewals not done online when an online option is available will have an additional processing fee (see “Additional Reg Fees” below), and fingerprinting will be required prior to the submission of a registration (see “Fingerprints” below).  As such, DSHS hardcopy forms will not be accepted for registration (renewal) at the TMB.

 

CRIMINAL HISTORY BACKGROUND CHECKS:

  • Criminal history background checks through the Department of Public Safety and the Federal Bureau of Investigation are required for all current licensees and permit holders. TMB has begun the process of fingerprinting existing licensees and notifications regarding this process will go out to all those effected by this change in early 2016.
  • The fingerprint background check will be required prior to the completion of your initial TMB registration/renewal, per the new statutory requirement set out in Texas Occupations Code, Sec 603.3031.

Note that the fingerprint results will not be REQUIRED until your renewal. Reminders of the fingerprint requirement will be included in the renewal notice sent out 90 days prior to your current expiration date.

 

  • A Board issued 6 digit ID number* is required for submitting your fingerprints. This 6 digit ID number is normally assigned at the time of application submission. For those who hold an existing permit, you will not be assigned a TMB 6 digit ID number until the transition of data from DSHS is complete. To expedite the processing of your results, please use the following general 6 digit ID “777777” to begin the fingerprint process.
  • Please note that the criminal history results can take 4-6 weeks for processing. Instructions getting your finger prints done are listed below under the tab “Fingerprints”.
  •  As results can take weeks to receive, we encourage you to being this process as soon as possible.
  • Per Federal regulations we are not able to access fingerprint results that were submitted to another agency. If you have been previously fingerprinted for another license or by another agency, we apologize for the inconvenience; however you will still be required to submit the fingerprint criminal history background prior to your initial registration/renewal at the TMB.

 

NEW REGISTRATION (RENEWAL) PROCESS

Existing license/permit expiration dates beyond March 31, 2016, will be honored, however no extensions will be provided at this time. Renewals will also continue to be done biennially.

 

Perfusionists Online Registration/Renewal – NEW!!!

 

PLEASE NOTE:

  • Registration/renewal through TMB will be completed on a different schedule than DSHS, with all permits expiring during specific set times of year (as opposed to expiring in the permit holder’s birth month). Moving forward perfusionist licensees will expire once a year, on August 31.
  • As part of the transition from DSHS to TMB, existing perfusionist licenses will have their first registration with TMB per the existing permit expiration date. The renewal fee charged at the time of the first renewal with TMB will be pro-rated to bring the permits in line with the new expiration dates.  
  • For example, perfusionist licenses that expire in December, or January of 2016 will be pro-rated to renew through August 2017, and licenses that expire in February, March, April or May of 2016 will be pro-rated to renew through August 2018.
  • For those licensees who have recently been issued or renewed with DSHS, it may be approximately two years before you are due to register/renew with the TMB.
  • As part of this transition, some existing license and permit numbers will be reformatted to fit TMB requirements.
  • Physician, Physician Assistant and several other licensure applicants are required to submit their fingerprints for state and national criminal history background checks.  Fingerprinting services are provided by MorphoTrust USA for a fee (see "Current fees" below), payable directly to MorphoTrust USA.  Results of the fingerprinting will be sent directly to TMB both from the Texas Department of Public Safety Criminal Records and the Federal Bureau of Investigation.

    Live Scan Fingerprinting – Only Available in Texas

    If you are in Texas, or reside out of state but can schedule a time to be in Texas, you may make an appointment for a live scan (a process that captures fingerprints without the use of ink), at locations throughout the state of Texas.

     

    1. Schedule an appointment to be electronically fingerprinted by MorphoTrust USA at one of their IdentoGo enrollment centers.

    • Internet based scheduling is the quickest and most convenient way to obtain a fingerprint appointment.
      1. You may begin the process now by simply clicking on this link: https://uenroll.identogo.com/servicecode/11G62J and then;
      2. click “Schedule or Manage Appointment”;
      3. complete all required fields on the following page;
      4. Enter your Board issued 6 digit ID#* with “MB-“ in front – example: MB-777777 
      5. Once you have scheduled your appointment, you are not required to bring the form to your visit.
    • If you prefer to schedule over the telephone, you must:
      1. First have the TX Fingerprint Service Code form (Fingerprint Service Code form);
      2. Then call 888.467.2080;
      3. Please have the TX Fingerprint Service Code form before you call –MorhphoTrust will prompt you for the Service Code (11G62J) on the form;
      4. Once you have scheduled your appointment, you are not required to bring the form to your visit.

    2. Arrive at your scheduled appointment with your photo identification and fee

    • MorphoTrust accepts Visa/MasterCard/Discover/American Express, business checks, money orders and coupon codes (employer accounts) at the time of service.
    • Please note that personal checks and cash are not accepted.

    3. Your fingerprints will be submitted electronically to DPS and the FBI. You will not receive a printed fingerprint card.

    4. At the conclusion of your appointment, the MorphoTrust enrollment agent will provide you with an IdentoGo receipt stating that you were fingerprinted.

    Fingerprints provided for this application shall be used to check criminal history records of the Texas Department of Public Safety and the Federal Bureau of Investigation, in accordance with applicable statutes.

    Please note: If you choose to schedule fingerprinting online, please ensure that you use the TMB service code as, by law, we cannot use fingerprint results that were sent to another agency. If you have accidentally selected another agency, prior to your appointment you will need to visit www.identigo.com, select the state where you would like to schedule your fingerprinting session, Online Scheduling, Begin Registration, and then select “I have an existing appointment I would like to change.”

     

    Hard-Copy Fingerprinting – For Use Outside of Texas

    If you reside outside of Texas and cannot schedule an appointment in Texas for a Live Scan, you must be fingerprinted in ink on a card (only cards provided by the Texas Medical Board are acceptable) at a local law enforcement agency, and must then submit the card directly to MorphoTrust USA to be digitally scanned and processed.

     

    1. Request a fingerprint card by contacting the Texas Medical Board at Screen-CIC@tmb.state.tx.us if you are an applicant, or at Registrations@tmb.state.tx.us if you are a licensee.

    2. Pre-enroll with MorphoTrust USA to submit fingerprint cards.

    • Internet based pre-enrollment is the quickest and most convenient way to submit fingerprint cards.
      1. You may begin the process now by simply clicking on this link: https://uenroll.identogo.com/servicecode/11G62J and then;
      2. Click “Submit a Fingerprint Card by Mail”;
      3. complete all required fields on the following page;
      4. Enter your Board issued 6 digit ID#* with “MB-“ in front – example: MB-777777
      5. complete all required fields on the following pages.
      6. On the next screen, click the “Pay for Ink Card Submission” button and complete all required fields on the following page;
      7. Complete payment screen;
      8. Print the confirmation document containing a bar code and complete by signing the waiver and filling in contact information;
      9. Once you have obtained your fingerprint cards, follow mail-in directions on the confirmation document.
    • If you prefer to pre-enroll over the telephone, you must:
      1. First have the TX Fingerprint Service Code form (Fingerprint Service Code form );
      2. Then call 888.467.2080
      3. Please have the TX Fingerprint Service Code form before you call –MorhphoTrust will prompt you for the Service Code (11G62J) on the form;
      4. Inform the MorhpoTrust representative that you wish to pre-enroll for a “hard card submission”;
      5. Once payment is complete a summary confirmation document will be emailed to you;
      6. Print the confirmation document and complete by signing the waiver and filling in contact information;
      7. Once you have obtained your fingerprint cards, follow mail-in directions on the confirmation page.

    3. Obtain a copy of your fingerprints by a criminal law enforcement agency on an original FBI APPLICANT fingerprint card. ALL requested information must be provided on the fingerprint card and you and the official taking the fingerprints must sign the card. ONLY CARDS PROVIDED BY THE TEXAS MEDICAL BOARD ARE ACCEPTABLE.

    4. All fingerprints MUST be captured by a law enforcement agency.

    5. Once you have obtained your fingerprint cards, follow the mail-in directions found on the MorphoTrust Pre-Enrollment Confirmation Page that you previously printed and completed.

    6. Wait for a receipt from MorphoTrust USA.

    Fingerprints provided for this application shall be used to check criminal history records of the Texas Department of Public Safety and the Federal Bureau of Investigation, in accordance with applicable statutes.

     

     

    Fingerprinting – Current fees

    As of 10/01/2016 the fee for fingerprinting is $38.50.  This fee includes the fingerprint vendor processing fee, DPS processing fee and FBI processing fee.

     

    * What is a Board issued 6-digit ID#?
    This number, assigned to you at the time you submitted your application for licensure, for physicians can be found online through LIST (Licensure Inquiry System of Texas) or for all other applicants through contacting the Texas Medical Board Consumer and Application Resources (CAR) Department at (512) 305-7030 between 8 a.m. and 5 p.m. Central Time.

    The full (24 months) biennial registration fee includes a $2 Office of Patient Protection (OPP) fee. This fee is required by statute. The remaining $360 is the fee required by TMB.

    Online Registration:

    No financial information is seen, processed, or stored by the Texas Medical Board. The payment portion of the online registration system is handled by Texas.gov, the official website of Texas. The price of this service includes funds that support the ongoing operations and enhancements of Texas.gov, which is provided by a third party in partnership with the State. Texas.gov will remit the amount paid to the Texas Medical Board on your behalf.

     

    Please note that as of 9/1/15, Texas.gov will charge a different amount depending on the type of online payment service used.

    Non-online (paper) registration:

    An additional processing fee of $50 will be charged to those licensees who choose to register through a paper form, when an online option is available. If an online option is not available (ex: license is delinquent or suspended at the time of registration) the additional processing fee will not be required.

    There is no grace period for the expiration date of a Perfusionist permit. Once the expiration date has occurred, a penalty fee equal to one fourth of the registration fee will be added. After 90 days, the penalty fee will increase to equal one half of the registration fee. If a license has been expired for one year or longer it is automatically cancelled.

    Please note that the Board does not have a “zero balance receipt” that shows that a licensee has a zero balance or that licensee registration has been paid in full, nor can we provide an individualized billing statement of the amount due to complete license registration/renewal.

    Please remember that the payment portion of the online registration system is handled by Texas.gov, the official website of Texas.  A printable receipt will be provided at the end of the online registration process. In addition, a confirmation of payment receipt will be emailed to the address given as part of the Texas.gov payment process.

    Additional copies of the receipt will be provided by request only. Submit your request to Registrations@tmb.state.tx.us . Please include your full name, license number, date of payment, and amount paid.
