PIT Apply
Apply online. You must have an ID number from your program to apply.
On the next business day, check for an e-mail from TMB
If additional information was requested, find the Supplemental Licensure Forms you need and submit them.

Get this from your program before you apply:
Your TMB personal ID number
The third party identification number for your residency program (only needed if they will be paying your application fee)
