Senate Bill 202 transferred four occupational regulatory programs from the Department of State Health Services (DSHS) to the Texas Medical Board (TMB). Medical Physicists, Medical Radiologic Technologists, Perfusionists, and Respiratory Care Practitioners are now regulated by the TMB and each occupation’s respective board/advisory committee.

If you have an active permit that was issued by DSHS, or if you have a permit that was issued by DSHS that has been expired for less than one year, you do not need to apply for a new one through the TMB.

As of October 7th, 2015 the TMB is accepting NEW applications for the Perfusionists. During this transition process, please check our website regularly for updates.
The Licensure Department of the Texas Medical Board is committed to furthering the mission of the Board by ensuring quality healthcare for the citizens of Texas by upholding the standards set in statute and Board Rule, and by efficiently processing licensure applications.
Please note that Texas has a two-step application process – pre-licensure and licensure. During pre-licensure, the applicant’s documents are collected and the applicant will be updated as to which documents have been received and are missing through regular mail or e-mail, if an address has been provided. Only after the initial documents have been received will your application be forwarded to the licensure step for processing. During licensure, a licensure analyst will review and verify all the information contained in the application and the supporting documentation. Additional information will be requested based on the review of your application and documentation.
Once you have submitted your application online, you will be contacted regarding supplemental documentation requirements. Depending on your answers, some of the questions on the application will require you to download a supplemental form and submit it. Some of the forms must be sent to a third party, such as your liability carrier. Applicants must also take and pass the Jurisprudence Exam and submit their fingerprints for state and national criminal history background checks.
Your application will not be processed until the following documents have been received:
Individual application processing time will vary based on the complexity of the application. The processing time clock begins when the applicant has submitted all initial documents and has moved to the second step. Factors that increase the processing time are answering “yes” to any of the Professionalism questions on the application.
Applications are submitted online, but additional information and supplemental forms may be submitted using the US Postal Service or a private overnight delivery service. Board staff will communicate with applicants by e-mail, if an address is provided, or by letter using the US Postal Service.
Application and Application-Related Fees
Application Fee: The current fee for perfusionist application in Texas is $180.00. The entire fee must be submitted before your application can be assigned and processed.
The application fee includes a $5 Office of Patient Protection fee. An additional non-refundable surcharge related to the Texas Physician Health Program ($5.25) will be assessed with the application fee. These fees are required by statute. The remaining $175 is the fee required by the TMB.

No financial information is seen, processed, or stored by the Texas Medical Board.
The payment portion of the online application system is handled by Texas.gov, the official website of Texas. The price of this service includes funds that support the ongoing operations and enhancements of Texas.gov, which is provided by a third party in partnership with the State. Texas.gov will remit the amount paid to the Texas Medical Board on your behalf.
Please note that as of 9/1/2015 Texas.gov will charge a different amount depending on the type of online payment service used.

Physicians, Physician Assistants, and several other types of licensure applicants are required to submit their fingerprints for state and national criminal history background checks.
Please note that having a criminal history does not automatically disqualify you from obtaining a license through the Texas Medical Board. For additional information please review the Texas State Auditor’s Guide to Applying for an Occupational License After Conviction or Deferred Adjudication.
Fingerprinting services are provided through IdentoGo by IDEMIA (formerly known as MorphoTrust USA, LLC) for a fee (see the Fingerprinting FAQs below for the current fees), payable directly to IdentoGo. Results of the fingerprinting will be sent directly to TMB from both the Texas Department of Public Safety Criminal Records and the Federal Bureau of Investigation.
Please be advised that taking a photo is part of the fingerprinting process and a requirement. The Department of Public Safety has requested that IdentoGo maintain a 6’ distance when possible and prior to starting the appointment and that they inform the applicant it will be necessary to remove their mask for a photo. If you choose not to remove your mask, the appointment may not proceed and you will be asked to reschedule your appointment for a later date.
Please note – applicants for initial licensure should not submit fingerprints for review until after they have submitted the online application. Additionally, fingerprint cards should not be mailed to the TMB.

As a way of increasing efficiency in existing processes, while reducing costs, the Board has been developing innovations to its online licensing systems to create a better user experience with added features.
The Board is pleased to announce it is transitioning to electronic licenses for a more paperless experience and to allow for enhanced licensee control.
As of September 1, 2019, the Board will no longer issue paper licenses upon issuance for the following license types:
What does this mean for licensees issued on or after September 1, 2019?
Once your license is issued, please allow 2 business days for processing. After that time you will be able to log into your MyTMB account and view, save or print a copy of your active license. In addition, after September 1, 2019, you will be able to use the same system to view, save or print a copy of your active license as needed.
Please note that primary source verification regarding a licensure status is always available to anyone, including licensees’ employers, via the TMB website through Look up a License.
What does this mean for licensees issued on or after September 1, 2019, who are not on the list above?
The following license types have 90 days to register and activate your license, once the Board has issued you an official license number. After September 1, 2019 you will be able to log into your MyTMB account and view, save or print a copy of your active license, once you have completed this initial registration of your new license. Please allow 2 business days for processing.
