PA Supervised Temporary License Log
If you are granted a supervised temporary license by the Licensure Committee or the Executive Director to meet active practice requirements you will be required to submit a log of the time worked. This log may also be used when the Licensure Committee recommends that you shadow for a certain amount of time. Make as many copies as needed. Your supervising physician must sign each page.
